WHAT IS OIL MAJOR INSPECTION
adalah, inspeksi dilakukan oleh oil major atau beberapa perusahaan minyak dunia untuk ketentuan standard safety kapal apakah memenuhi syarat yg ditentukan oleh IMO sehingga kapal layak memuat muatan hasil produksi perusahan minyak tsb dan sebagai jaminan untuk Konsumen pengguna jasa angkutan laut dimana muatan yg dimuat diatas kapal bersangkutan di tangani oleh orang kapal yg mengerti sehingga keamanan muatan dapat terjamin

Apakah yg di periksa ?
- Melakukan pemeriksaan diatas kapal secara mendetail tentang ‘Hull (lambung) dan Struktur’(bangunan kapal)
- Melakukan pemeriksaan diatas kapal secara mendetail tentang ‘maintenance plan’ atau rancangan / rencana pemeliharaan kapal secara keseluruhan
- Melakukan pemeriksaan diatas kapal secara mendetail tentang ‘ISM compliance
- Melakukan pemeriksaan diatas kapal secara mendetail tentang ‘Maintenance & testing of equipment dan record
- Melakukan pemeriksaan diatas kapal secara mendetail tentang ‘drill and training’sehubungan dengan SOLAS , MARPOL , ISM dan ISPS
- Melakukan pemeriksaan diatas kapal secara mendetail tentang dokumentasi FILING system sehubungan dengan beberapa hal tsb diatas sesuai dengan SMS
- Melakukan pemeriksaan diatas kapal secara mendetail tentang ‘certification of vessel and crew documents.
- Melakukan evaluasi tentang kemampuan / pengetahuan crew mengenai safety dan pengoperasian kapal dengan aman dan menjaga lingkungan dari pencemaran

SHIP INSPECTION Report
Apa saja yang diperiksa ?
BRIDGE.
- NTM terakhir harus ada diatas kapal.
- Peta harus lengkap dan dikoreksi sesuai NTM terakhir.
- Passage planning dibuat dari BERTH to BERTH.
- Penentuan posisi dilakuakan dgn Lebih dari satu METHODE
- Kesalahan pedoman harus selalu di check atau sekali setiap jaga
BRIDGE.
- Publications ALRS, Pilot Book, List of Lights, are correct up to latest NTM.
- Large scale chart are in use for concerned area
- Temporary and Preliminary notices file are available on board corrected on chart
- Compass error observation carry out at least once per watch.
BRIDGE.
- check list of preparation to sea.
- check list of familiarization with bridge equipments.
- Master/Pilot information exchange procedure are in use.
- Magnetic and gyro compasses are compared at least every hour and keep record.
- Actual readings from ship magnetic compass are in reasonable range comparing with deviation book
BRIDGE.
- A Magnetic compass deviation table is available on bridge ( for one year, curve within +2 ยบ).
- If celestial observation are carried out for the position fixing, the evidence such as calculation note book are available
BRIDGE.
- Check list of preparation for arrival/departure port.
- Check list of pilotage and passage plan appraisal.
- Bridge equipments maintaining log
- Chart and publication list must keep up to date.
- Bridge log book must use by pen.
- Echo sounder is operated when making a landfall and coastal water, date and time of switching on of Echo sounder shall be marking in recording paper or other separate log
BRIDGE.
- GMDSS log book is recorded properly and up-to-date.
- Testing of GMDSS equipment before departure
- Testing of astern of propulsion 30 minutes before entering the port.
- Testing of steering 12 hours before departure is recorded.
- Exchange procedure for master assume control the bridge command should be recorded
H O S P I T A L
- Medicine are properly stored and controlled.
- Medicine inventory log is kept up to date (without expire).
- Hospital is clean and tidy ( including Shower room & toilet).
- Oxygen resuscitator appropriate level (150kg/cm²).
- Antidote for noxious cargo are listed up to date and available for control of present cargo
SAFETY.
- Emergency fire wire (F&A) are properly rigged and controlled according to tide and cargo loading/unloading.
- Windlass and winches (F&A) comings are plugged.
- Windlass and winches are secured by lock pints.
- Anchor chain (P&S) are locker with locking bar during port stay.
- During entering port both anchor should be ready for immediate use
SAFETY.
- Mooring lines are guided and tended properly.
- Safety net of gangway is properly rigged there is life buoy with the self igniting light and life line near the gangway.
- Pump room entry permit is issued and posted at entrance of pump room.
- Pump room rescue equipment are properly rigged for immediate use.
SAFETY.
- Oil spill gears such as empty drum, sawdust, cotton rug, non sparking scoop, wilden pump is available on deck.
- Wilden pump are connected to air line for immediate use in case of oil spill.
- Cargo manifolds oil spill drip tray free from oil.
- Drain cock of manifolds spill boxes are blinded.
SAFETY.
- Pipe line and flanges on deck are fully bolted.
- Accommodation air conditioning system is on recirculation during cargo operation.
- Emergency stop for accommodation ventilation system clearly marked and known to all crews.
- Cargo Oil ullage hatch cover and butterworth manholes are fully closed.
SAFETY
- Emergency response equipment such as portable fire extinguisher, fire hoses, protective clothes, SCBA, are prepared necessary near the cargo manifold and ready for use.
- Pilot ladder are in good condition.
- Officers and crews are familiar with operation of fire fighting, life saving and other emergency equipments.
Steering Gear
- Steering Gears must be clean and tidy.
- Communication equipment should available and equipped with compass if necessary
- All instruction (how to operated) must be displayed as Emergency fire pump , Compressor, and Steering gear, Emergency generator, Sewage plant , Life boat , Life raft , Medical oxygen , Fix CO2 system , Fix foam system , Fix dry powder system , and SOLAS Training manual , Fire Detection Manual shall available in the mess room or appropriate place
Accommodation
- Accommodation must be clean and tidy including, TOILET, GALLEY, MESS ROOMS.
- Accommodation must be free from cockroach.
- Fireman outfit including BA set are good condition (must be completed) and ready for used.
- Emergency fire plan must be posted in the alleyway and also in near the gangway port and starboard.
Pump Room
- Pump Room must be free from, OIL or WATER and pipe lines leak.
- Bilge alarm must be marked in place.
- Sea Chest must be lock and chain and must be tested in good order by regularly checked , Pollution Prevention warning shall in place
Emergency Battery Room
Battery Room must be fitted with,
- Google, GAS MASK,
- RUBBER GLOVE, APRON
- RUBBER BOOT, and
- Liquid test kid complete
Procedures how to check and recharge those battery shall displayed in place
Deck Stores
- All the deck stores must be clean and tidy.
- Paint store must be free of accumulation of gas , make it clean and tidy
- Oil Spill Gears are properly stored as maintained at the SOPEP/SMPEP to bestowed separately with other article.
- Bitter end in forecastle store are marked and Hammer is provided in place.
Hull Structure
- General hull condition is satisfactory.
- Load line, Draft, Tug push, are clearly mark.
- The hull must be free from sign of oil, and dented.
Cargo Operation
- MSDS are readily available/posted for all product being handled,
- Max. loading rate, venting capacities, vapor pressure table are posted in CCR.
- Vessel is capable of operating in close condition.
- Cargo loading/unloading plan is available and posted in CCR with signature of concerned crew.
- Ship shore safety checklist, including part B of bulk – liquid chemical has been completed correctly.
- Test records of flammable atmospheres in non-cargo space are keep accordingly.
Officer in cargo duty are familiar with;
1). Innerting, padding and draying.
2). Reactive of cargoes.
3). Corrosive of cargoes.
4). High density cargoes,
5). Solidifying and high viscosity cargoes.
6). Static Electricity.
7). Cargo Compatibility chart and posted in CCR
Cargo manifold spill box capacity and Cargo lifting gear capacity shall be clearly marked
Engine Room
- Appliance and housekeeping condition are satisfactory (VIQ 12.10).
- Main switch board, alternators and other electrical equipment are protected (VIQ. 12.4).
- Engine room is free from flammable source such as paint can or oily rags (VIQ 12.1.4).
- Pipe line and pumps are free from leaks (VIQ 12.11.6.6).
- Maintenance records are kept (VIQ 12.3).
- Bilge are free from oil and water (VIQ 12.9.8).
- Electric welding equipment is in good condition and written safety guidance are available (5.8.2).
- Engine room bilge ODM in good working under MARPOL annex I/regulation 16 (VIQ 6.10).
- C/E standing order is posted in engine control room.
- Standing instruction for Emergency Generator are clearly displayed (VIQ 12.6.4).
- Lifting derrick are checked periodically and the record must be kept on board.
